Your items will be assigned a work order number upon receipt by IVS, no RMA is needed.

When shipping your equipment in for service, please enclose a packing slip or purchase order
with each box.  Packing slips should include the following:

a. Attention: Repair Department 
b. Company Information:

• Name 
• Address 
• Telephone Number 
• Contact Person 
• Email Address 

c. A list of items to be serviced with a detailed problem/failure description 
d. Any special Instructions 
Unless otherwise specified, all units will be returned via UPS ground.  You may specify a
particular service or carrier.  To do so please specify on your shipper or purchase order.
